Running total and running AOV per customer

For each PAID order in time order per customer, return: - the running total of amount_usd so far - the running average order value (AOV) so far

Sort by customer_id then order_ts ascending. Round running_avg to 2 decimal places. Refunded/cancelled/delivered/ NULL orders don't count.

Columns: customer_id, order_id, order_ts::DATE AS order_date, amount_usd, running_total, running_avg.
