Label every order by size:

medium — 100 or more (but under 200) - •
small — under 100 - •
unknown — amount_usd is missing (NULL)
An order with no amount must not be reported as small — finance wants those flagged so someone fixes the load.
Return order_id, amount_usd, size. Sort by order_id.
