checkout_orders (order_id, customer_id, item_total, shipping_fee) holds the shop's orders. Orders that shipped free were saved with no shipping fee at all: their shipping_fee is NULL.
Return every order's grand_total: the item total plus the shipping fee, where a missing shipping fee counts as 0. Every order must appear.
Columns: order_id, grand_total. Sort by order_id.
